Make sure you connected yellow wire coming from t-harness to yellow wire on the 20 pin connector of the evoall.

is the yellow wire showing 12v when ignition is on and 0v when ignition is off ?

Yes it does show 12v on and 0v when off

Master reset and reprogram

Hold button, insert 4 pin connector, release on RED led, press again program button and hold until leds start to alternate then release. This is master reset procedure, now disconnect 4 pin and reprogram your module.
